You’ve probably heard the buzz around NAD+ — IV drips, injections, patches, and supplements promising more energy, sharper focus, better recovery, and slower aging.

There is real science behind the interest. NAD+—short for nicotinamide adenine dinucleotide—is a coenzyme found in every cell of the body. It plays an essential role in converting nutrients into usable cellular energy and supports enzymes involved in DNA repair, stress responses, and metabolism. NAD+ availability also tends to decline with age and in certain metabolic conditions.

But here’s what the hype often leaves out:

NAD+ Is Not A One-Size-Fits-All Solution

The question is not simply, “Does NAD+ work?” A better question is: Is NAD+ the right support for your body, at this point in time—and in the right form and dose?

That answer depends on more than your symptoms. It can depend on your overall health picture: inflammation, gut function, nutrient status, medications, genetics, toxic exposures, and how your body handles methylation.

When those systems are under strain, adding a new supplement or injectable does not always produce the result someone expects. A person may be hoping for more energy, but if they are dealing with poor sleep, inflammation, nutrient deficiencies, or unresolved digestive issues, NAD+ alone may not address the real bottleneck.

That does not mean NAD+ is “bad.” It means the foundation may need attention, too.

Toxic Load Is Part Of The Picture

We are all exposed to environmental compounds through air, water, food, work, personal-care products, and household products. Depending on someone’s history and current environment, potential exposures can include mold-related compounds, heavy metals, pesticides, solvents, and other chemicals.

The body has natural systems for processing and eliminating many substances. However, when exposure is significant—or when a person’s nutritional status, liver function, gut health, or genetics make processing more difficult—those systems may be under greater demand.

For some patients, exploring potential exposures and supporting foundational health may be more useful before—or alongside—NAD+ support. The goal is not to blame every symptom on “toxins.” The goal is to understand the full picture instead of assuming a single therapy will solve everything.

The Bigger Picture Behind NAD+

Why Methylation Matters

Methylation is a normal biochemical process happening throughout the body, every day. It helps regulate gene activity, supports neurotransmitter metabolism, participates in detoxification pathways, and helps the body process certain nutrients and compounds.

A simple way to think about it: methylation helps the body manage, recycle, and move things through.

One important marker connected to this process is homocysteine. Homocysteine is a naturally occurring amino acid produced during normal metabolism. The body typically recycles it through pathways that depend on nutrients such as folate, vitamin B12, vitamin B6, and riboflavin.

When homocysteine is elevated, it does not diagnose one specific problem. However, it can be a useful clue that the body may need a closer look at nutrient status, methylation-related pathways, kidney function, medication effects, lifestyle factors, and cardiovascular risk.

How NAD+ Can Interact With Methylation

This is where nuance matters.

Some NAD+ products are precursors, meaning the body converts them into NAD+ through metabolic pathways. Certain forms—including nicotinamide-related forms—can ultimately be metabolized through pathways that use methyl groups. In theory, high methyl-group demand could place added pressure on methylation pathways in certain individuals.

That does not mean everyone who uses NAD+ will have a methylation problem. In fact, a short-term human study of high-dose nicotinamide riboside found no harmful effect on global DNA methylation or homocysteine, including among participants with common MTHFR variants. Still, people do not all respond to supplements or injectables in the same way. Form, dose, duration, baseline nutrition, medications, health conditions, and individual sensitivity can all matter.

For someone who is already sensitive to stimulating supplements or who has underlying nutrient or metabolic issues, NAD+ support may feel less than ideal. They might report symptoms such as:

  • Feeling wired or restless

  • Increased anxiety or irritability

  • Difficulty sleeping

  • Headaches

  • Brain fog or feeling “off”

Those symptoms are not proof of “overmethylation,” which is not a universally accepted medical diagnosis. Rather, they are a sign that the product, dose, timing, or overall approach may need to be reconsidered with a qualified practitioner.

Is NAD+ Right For You?

Test Before You Guess

The purpose of testing is not to create more complexity. It is to avoid blindly adding another wellness trend to an already overloaded system.

Depending on your history, symptoms, and goals, a practitioner may consider testing or reviewing areas such as:

  • Homocysteine

  • Folate, vitamin B12, vitamin B6, and related nutrient markers

  • Genetic variants that may inform nutrient processing

  • Metabolic and inflammatory markers

  • Gut-health indicators

  • Potential environmental exposures, when clinically appropriate

  • Medication and supplement history

No single test can tell you whether NAD+ is “right” for you. But thoughtful testing can reveal patterns, identify priorities, and help guide a more individualized plan.

So, Is NAD+ Right For You?

Possibly—but not automatically.

NAD+ may be worth considering for people who want to explore support for cellular energy and metabolic health, particularly when it is part of a broader, individualized plan. But it should not replace the basics: quality sleep, adequate nutrition, movement, stress support, a healthy gut, and addressing medical concerns with the appropriate clinician.

The best results tend to come from asking the right questions first:

  • What is my body already dealing with?

  • Are there foundational issues that need attention?

  • Which form of NAD+ makes the most sense for me?

  • What dose and timing are appropriate?

  • Are there markers worth checking before and during use?
